  1. Meeting Preliminaries

    • Meeting Opened / Attendance Taken (see above).

    • - Call meeting to order and review minutes of the previous meeting.

    • Pumping Reports - 32 Laurel Hill Dr., 121 Cave Hill, 172 Shutesbury, 113 Juggler Meadow,Main House Complex 1, 113 Juggler Meadow, Car Garage, 102 Dudleyville Road, 74 Amherst, 87 Depot, 180 RSG, 63 Long Plain, 47 Camp, 35 Amherst, 5 Montague.

    • - 1. Received from Greg's Waste Water Removal Title 5 Inspection Report dated 5/23/2019 for 295 Shutesbury Road., owner Barbara Raymond. System fails. Received check#349, for $150.00 for Title 5. Board decided that to increase flow property shall be fully compliant. i.e. a reserve area must be available

      Steve Ball, Health Agent arrived at 7:34pm.
      295 Shutesbury Road needs system repair of existing 2 bedroom trailer. Discussed whether to enforce 3 bedroom minimum design or continue as a 2 bedroom design with a deed restriction and the ability to show a reserve area.

    • - Citizens business: Applications, permits and general questions for the board.

    • - Received a letter from GMG Enterprises dated 5/28/2019 regarding Camp Rd., Map A5, Lot 3. The letter states that Greg's WasteWater Removal will be pumping the holding tank designed by William J. Sieruta on an as-needed basis.

    • - Received DSCP Application for 70 Camp Rd., Map 5A, Lot 3, owner Thomas Friedman. Tight Tank. (Fee NOT received) Assigned DSCP#R2019-06-03a. It was noted that the builder should get a correct address from the Town Clerk before things continue at the property.

    • - Received a letter from Steve Ball, HA., dated 5/29/2019 regarding 70 Camp Rd., Map 5A, Lot3. A repair of an existing system Installation of a 2200 gallon tight tank. The letter states that the plan appears to meet the requirements of 310 CMR 15.000 the state environmentalcode and Leverett local regulations.

    • - Received from Complete Septic Services, LLC Title 5 Inspection Report dated 5/3/2019 for 258 Pratt Corner Rd., owner John Frost. System passes. Fee NOT received.

    • - Received COC Certificate of Compliance dated 5/28/2019 for 63 Teawaddle Hill Road. Yellow Brick Management Inc. Signed by Inspector Stephen Ball, HA.

    • - Received from WilliamJ. Sieruta, Title 5 Inspection Report dated 5/23/2019 for 32 Laurel Hill Dr., owner Janet Straus. System passes. Fee has been paid.

    • - Received a copy of a certified letter return receipt requested to Joshua Kenny 48 Meeting House Rd., Pelham, MA regarding 456B Long Plain Road. Notice of Code Violations Order to Correct dated 5/22/2019. Numerous violation sited. A re-inspection of the property will be conducted on June 19, 2010 @ 12 Noon.

    • - Received Check#1016, dated 6/3/2019 for $150.00 from Anna Sloan owner 33 Long Plain Road. Title 5 Inspection. Conditional Pass. New Tank and D Box needed.
